In recent years, the allure of “temporary” tattoos, such as those offered by Ephemeral, has captivated many. Promising designs that fade within a year, these tattoos seemed like the perfect commitment-free body art. However, as highlighted in a recent USA Today article, numerous individuals find themselves with lingering ink, leading to unexpected tattoo regret.
At Cosmetic Laser Dermatology (CLDerm) in San Diego, we’ve been at the forefront of laser tattoo removal for over three decades. Our Medical Director, Mitchel P. Goldmsan, MD, actually helped develop the first laser specifically designed to remove tattoos in 1992. Our pioneering work in laser technologies and the development of the TotalTat³™ protocol positions us uniquely to address even the most stubborn tattoos.
The Challenge with “Fading” Tattoos
Ephemeral tattoos were designed to degrade naturally over time. However, factors such as ink composition, depth of application, individual skin types, and immune responses can influence the fading process. Consequently, many find their tattoos persisting far beyond the anticipated timeframe.
Choosing the right laser for tattoo removal isn’t one-size-fits-all. Different wavelengths and pulse durations work better for certain inks and skin types, which is why expert evaluation is so important.

CLDerm’s Legacy in Tattoo Removal
Our commitment to advancing tattoo removal techniques is well-documented:
- In 1994, Dr. Mitchel Goldman co-authored a seminal study on the efficacy of the Q-switched alexandrite laser for tattoo removal.
“The alexandrite laser is a safe and effective treatment modality for removal of black and blue-black tattoo pigment.”
— Fitzpatrick RE, Goldman MP. PubMed - Recognizing the limitations of single-wavelength treatments, we developed the TotalTat³™ protocol, a comprehensive approach combining multiple laser technologies to optimize results.

Introducing TotalTat³: A Comprehensive Tattoo Removal Solution

The TotalTat³™ protocol is a multi-faceted treatment approach tailored to individual tattoo characteristics and patient skin types. It integrates:
- 755nm Alexandrite Laser
- Fractionated CO₂ Laser Resurfacing
- PicoSure®, PicoWay®, Q-switched Ruby
This technology stack allows us to treat virtually every ink color and skin tone safely and effectively—often with better clearance in fewer sessions than standard approaches. TotalTat³™ is designed to eliminate ink faster and more thoroughly, restoring your skin’s natural appearance with less time and discomfort.
“Picosecond laser is an effective and safe treatment for cosmetic tattoos… unmasking of an orange color postlaser treatment can be successfully treated with a 532-nm PS laser.”
— Hartman N, Loyal J, Borsack S, Goldman MP, Boen. Retrospective Review of the Laser Removal of Facial Cosmetic Tattoos. PubMed
We also offer complimentary removal of radiation marker tattoos for patients who have undergone cancer treatment.
What to Expect During Treatment
Tattoo removal at other practices can be painful—but at CLDerm, patient comfort is our priority. We go above and beyond to maximize comfort using cold air, lidocaine-based anesthesia options, nitrous oxide (laughing gas) and eye shields for sensitive areas. Most patients describe the sensation as painless or a brief rubber-band snap, depending on the anesthesia used..
Treatment time depends on tattoo size. Afterward, you may notice redness, swelling, or temporary pigment changes. These typically resolve over time. For best results, we space sessions 4–6 weeks apart and recommend 3–5 total treatments, depending on tattoo complexity.
Clinical Outcomes and Patient Satisfaction
Our clinical studies demonstrate the efficacy of the TotalTat³™ protocol:
- In a review of 33 patients undergoing facial cosmetic tattoo removal:
- 66% achieved over 75% clearance in just three sessions.
- 34% saw 51–75% clearance.
- No cases of scarring or long-term complications were reported.
- — Hartman N, Loyal J, Borsack S, Goldman MP, Boen. PubMed
- Another study highlighted the benefits of combining picosecond lasers with fractionated CO₂ ablation:
“This study shows a significant decrease in bulla formation associated with tattoo treatment when fractionated CO₂ ablation is added to the picosecond Alexandrite laser.”
— Au S, Liolios AM, Goldman MP. ResearchGate
Addressing Cosmetic and Semi-Permanent Tattoos
Cosmetic tattoo removal—including microblading, eyeliner, lip liner, and lip blush—requires extra precision due to pigment complexity and skin sensitivity. TotalTat³™ is safe and highly effective for these areas.
- A study on eyebrow tattoo removal using picosecond Nd:YAG lasers reported:
- 70% of patients experienced 75–100% clearance.
- No adverse effects like scarring or pigment changes.
- — Pawar M, Satvaldinova A, Singh M. ResearchGate
Laser removal of eyeliner and eyebrow tattoos can be both safe and highly effective—when done correctly. Proper eye protection, pain management, and planning for potential pigment changes are all part of a well-executed treatment.
