Collagen and elastin are crucial to the health and appearance of human skin. Both substances help ward off skin laxity by fighting the effects of gravity for tight, firm skin that is also supple and pliable. However, when they begin to break down, the result is thinning, drooping, loose skin.
Volume loss in skin affects both women and men, usually causing a more aged appearance. Cosmetic Laser Dermatology offers cutting-edge non-surgical treatments to address skin laxity and volume loss that can be customized to meet the needs of each patient. Our board-certified dermatologists have extensive experience with the latest and most effective treatment options for aging skin and are considered leading experts in cosmetic dermatology. Led by esteemed cosmetic dermatologist Dr. Mitchel Goldman, the doctors at CLDerm have been involved with many of the clinical trials and research studies behind the following dermatologic solutions to skin laxity and volume loss.

Juvéderm is one of the most popular and effective dermal filler treatments available at our San Diego practice. The injections are composed of hyaluronic acid (HA), a hydrating substance found naturally in the body, which smooths out wrinkles and folds by restoring volume loss in the skin. There is also Juvéderm Ultra Plus, which is a thicker version of the traditional Juvéderm, making it very effective for softening deeper lines, such as nasolabial folds.
Juvéderm Voluma is another FDA-approved filler from the leading pharmaceutical company Allergan that is specifically designed to volumize the mid-face, cheek, and chin areas. As a thicker HA filler, Voluma can provide immediate results that last up to 2 years or more.
The latest Juvéderm HA filler to receive FDA approval is called Juvéderm Vollure XC. Vollure is an ideal filler treatment for moderate to severe facial wrinkles, such as nasolabial folds. Vollure is a true game-changer in cosmetic dermatology as the results can last up to 18 months.
Restylane injectable treatments improve volume loss in the skin by diminishing wrinkles and other signs of aging. Also composed of hyaluronic acid, Restylane creates more volume and moisture in the skin. The improvement is gradual and subtle, which is why it is also effective for restoring volume to lips that have thinned due to aging.
The Restylane line of fillers recently added two new formulations that are considered “next generation” HA fillers to correct moderate to severe wrinkles, particularly the nasolabial folds. Restylane Refyne and Restylane Defyne are innovative fillers that incorporate a new technology called XpresHAn, with creates a uniquely flexible quality to the injectable gel that mimics the skin’s natural movement. With Refyne and Defyne, patients can effectively erase laugh lines and maintain natural-looking results for up to 12 months.
Many people don’t realize it, but the hands often betray one’s age. As the skin loses volume, the bones and veins become exposed and very prominent. Radiesse is a calcium-based filler that is the only filler FDA-approved specifically for the hands. Not only are the injections highly effective for treating skin laxity, but they also stimulate collagen production in the skin to add lift and volume over time. The skin immediately looks rejuvenated yet continues improving for 3-4 months as collagen production is increased. Peak results happen after about six months and can last up to a year or more.
Sculptra is another collagen builder that delivers a gradual improvement from volume loss and skin laxity. It is a common anti-aging treatment for its ability to correct the appearance of volume loss as well as add contour to the face. Usually 3-5 separate, monthly treatments are required to achieve optimal results that can last from 2-3 years.

Non-Invasive Skin Tightening
Neither skin laxity nor volume loss is restricted to the mouth or cheeks. It also occurs in the eyebrows and the neck. Ulthera, commonly referred to as Ultherapy, is a non-surgical skin lifting treatment that utilizes ultrasound energy to go deep to restore volume and tighten loose skin for smoother, more elastic skin.
For patients averse to surgery, such as a facelift or neck lift, Ultherapy is a viable skin tightening alternative. For patients with minimal skin laxity, it is an effective preventative measure. The board-certified dermatologists at CLDerm actually played an integral role in achieving FDA approval for using Ultherapy for wrinkles of the decolletage or upper chest.
Another non-surgical skin volumizer is Silhouette InstaLift, which lifts skin and restores facial volume using dissolvable sutures. The treatment also goes deep to increase the body’s natural rate of collagen production in the treated area. Patients experience noticeably restored volume that gradually improves; after 2-3 months, the effects will be visible yet not dramatic. Results last 1-2 years until increased skin laxity due to continued aging occurs.
The non-surgical Silhouette InstaLift means patients do not need to deal with incisions or anesthesia. In fact, this procedure involves specially-designed dissolvable sutures that slowly disintegrate as they simultaneously address deep tissue issues and revitalize skin laxity for a result that is natural-looking and customized for each patient. All five of our dermatologists are specially trained in performing the Silhouette InstaLift procedure to provide customized facial rejuvenation for our patients.
With targeted radiofrequency energy, ThermiLift is excellent for non-surgical fat contouring and skin tightening. It is an alternative to a face or neck lift, though without the accompanying incisions and scarring.
FDA-approved ThermiLift reduces excess fat and skin laxity with little pain and minimal downtime for an effective choice in facial rejuvenation.
For minimally invasive skin tightening, ThermiTight sends radiofrequency energy into the deep layers of the skin, heating the tissue enough to stimulate collagen production without damaging healthy cells. This gentle heating action strengthens the foundational support of the skin and results in tighter, firmer skin.
To help trigger collagen production has slowed down due to aging, treatment with Thermage uses radiofrequency to heat up the layers deep within your skin. This helps the skin appear tighter and firmer, enhances skin tone, and improves skin texture.
A single session of non-invasive Thermage treatment is virtually painless and results in no downtime for an effective, non-invasive skin tightening alternative to liposuction. We recommend a second treatment in 6 months to achieve optimal results then consider additional treatments every 2 years or so.
A liquid facelift is designed for patients with minimal skin laxity or for patients looking for a non-surgical method of addressing volume loss in the skin around the mouth and cheeks. It restores a youthful appearance with virtually no downtime necessary.
A major advantage to a liquid facelift is that it can be customized with any combination of dermal fillers and neuromodulators to fit a patient’s given needs and goals. Our board-certified, experienced, and highly-skilled cosmetic dermatologists will working closely with a patient to determine his or her goals and create a custom treatment program to achieve those goals.

Body Laxity Treatments
For women experiencing minor breast ptosis (drooping or sagging), especially due to weight loss or after breastfeeding, a surgical breast augmentation procedure is typically excessive. ThermiBreast is a non-surgical procedure with radiofrequency technology that is gentler and, for minor breast ptosis, more effective. This procedure works best in women with smaller breasts (B-C cup).
ThermiBreast produces a noticeable lift to the breast for an effective non-surgical option for women with ptosis of an inch or less.
Women, especially after childbirth, are susceptible to weakened vaginal canals and stretched labia, which can be uncomfortable and affect sexual pleasure as well as control of urinary urges. ThermiVa uses the same radiofrequency technology that ThermiLift or ThermiTight do to rejuvenate lax vaginal tissue.
